- 2012 Application: The application is an MS Word document (.doc) which uses locked form fields. Please type your responses directly into the spaces provided.

The registration fee for the Course Design Institute is $825* and covers
- all programming costs
- print resources, including a program binder with worksheets and handouts, and Dee Fink’s bestselling book, Creating Significant Learning Experiences
- one-on-one consultation services during CDI
- lunch and refreshments each day of the Institute
Up to five UVa graduate students will be accepted to participate in the Institute, with all expenses covered by the Teaching Resource Center.
Payment & Registration: Once applicants are accepted to participant in CDI, they will receive detailed information about payment procedures and registering for the noncredit Course Design Institute (NCED 178). Payment can be made by check through UVa's School of Continuing & Professional Studies (SCPS) or online via e-check (no fee) or credit card (2.75% service fee, no Visa). Payment must be received by Friday, May 4.
Refund Policy:
A 100% tuition refund will be granted up to one week prior to the start of CDI. The refund period ends one week prior to the start of the course. Requests for refunds must be made in writing to SCPS in addition to notifying the Teaching Resource Center.
*UVa Faculty & the Education Benefit: The CDI qualifies for the UVa Education Benefit. The Education Benefit provides centrally-funded financial support for education, vocational, and professional development. If you are an UVa Academic Division employee (faculty, staff, and some post-doctoral designations) and have one-year of benefits-eligible service to the University, you have an annual $2000 Education Benefit which can be used to pay for this course. For more information on the benefit and to confirm eligibility, visit the University Human Resources (UHR) website. To access the online form to request your benefit, go to www.hr.virginia.edu and click on Benefits@ in the upper right corner. Once you have signed into your Benefits account, click on Education Benefits on the bottom left under "I would like to change my…" Employees paid on a biweekly basis will receive benefit funds within two pay periods of requesting the benefit; those paid monthly will receive benefit funds in their next paycheck. Use your benefit payment to pay for the course directly.
